Sonder Health Plans Taps Cleo for Care Coordination & Family Caregiving Support

Sonder Health Plans, a Georgia-based health plan provider, is partnering with family caregiving company Cleo to support Medicare Advantage members, the companies shared with MedCity News exclusively.

Sonder Health Plan serves 84 Georgia counties, including Macon, Columbus and Savannah. Working with San Francisco-based Cleo will help the insurer manage chronic conditions, address health-related social needs and direct members to the right care.

Cleo identifies which members have risk factors for physical and emotional well-being and provides tailored care plans to address their physical and mental health needs. Members and their families are connected with a guide, who is available via call or message. The company also provides care navigation support to connect members with providers and medical services. In addition, Cleo can identify and address non-medical needs, such as transportation, food insecurity and housing.

“We’re really taking that family-centered care approach, where you’re understanding what the needs are for that older adult, whether it’s across their cognitive health, their physical health, their mental health, their social health,” said Madhavi Vemireddy, CEO of Cleo, in an interview. “Are there any health-related social needs? Where can Cleo help connect the dots?”

The Cleo guide can also support caregivers, including by connecting them with mental health care and scheduling home safety assessments. This is especially important in today’s society where both parents are often working in addition to caring for a loved one, according to Dr. Dirk Slaker, chief medical officer of Sonder Health Plan.

“When I grew up in the 50s and 60s, there were single family incomes,” he said in an interview. “My mom never worked outside of the house, and my dad was a school teacher making very little money, but we were able to live a nice middle class existence. That really doesn’t occur much anymore. So in addition to just having to care for somebody, you have people that are [having their jobs impacted]. Somebody’s got to give something in that to be the caretaker. That makes having organizations like Cleo even that much more important for caretaker support.”

The partnership comes as the senior population is growing. By 2030, about one in five Americans will be 65 years old or older. In addition, 40% of the U.S. population is caring for an adult loved one.

To track the success of the partnership, Cleo will be following metrics like patient engagement and the company’s ability to address health-related social needs, Vemireddy said. On Sonder’s side, the health plan will be tracking emergency room visits, hospital readmissions and other metrics, according to Slaker.

Cleo isn’t the only company providing caregiving support. Others include Wellthy and Cariloop, though these are mostly focused on the commercial market, Vemireddy said.
